Junior Program Health Form and Waiver
ATIS requires all participants in its Junior Program to submit a completed health form and waiver once each season before their first trip.
Please complete the health form/ waiver online for each child before the start of the first trip. If you prefer to fill out a paper copy, come visit us at the ATIS building and we will have paper copies available.
